Documents Required to Submit Your Application

The body of the application is submitted via an on-line form. An optional worksheet containing the requested information is available for those who wish to compose their application in another format and then paste the information into the on-line form.

Electronic files required for uploading to the website depend on the type of agency submitting the proposal. PDF format is preferred for uploaded files to preserve formatting.

Nonprofit agencies will need:

  • Collaboration letter from lead veterinarian
  • Letter of support from director
  • Current fiscal year agency budget
  • IRS 501(c)3 determination letter
  • Current FL DOACS registration certificate (View Sample)
  • Most recent IRS 990, 990 EZ or 990N (If not for recently-completed fiscal year, submit proof of tax extension filing.)
  • Previous Year’s Financial Statement of Income & Expense

Municipal agencies will need:

  • Collaboration letter from lead veterinarian
  • Letter of support from director
  • Current fiscal year agency budget showing line-item for animal care and control
